Many couples struggle to have a baby. This struggle carries with it stress, hope, and uncertainty. Fortunately, the vast majority of fertility problems can be treated; either by treating the underlying cause or overcome using assisted reproductive techniques.

The cause of fertility problems may be related to the female partner, male partner, both partners, or in 20% of the cases remain unknown. Importantly, all of these causes are usually amenable to treatment.
The first step is to perform the basic fertility tests and imaging which involve making sure that both ovulation and sperm production are within normal, that the female genital tract is patent and healthy, and that the internal lining of the uterus is adequate for the embryo’s implantation and growth. Depending on the couple’s medical and surgical history, additional tests may be performed.

Most of the fertility problems are resolved using a short period of watchful waiting, minor treatments, or limited minimally invasive surgery. In case assisted reproduction is needed, then this should be planned in a way to optimize outcomes.
In-vitro fertilization (IVF) is one of the most effective assisted reproductive techniques. In IVF, the woman is given injections to stimulate the development of multiple eggs. Once the eggs are ready for ovulation (usually after around 10 days of injections), they are collected using a needle under ultrasound guidance and a light anesthetic. The collected eggs are then incubated or injected with the man’s sperm to fertilize in a controlled laboratory environment. Within two to three days, the fertilized eggs develop into embryos which are transferred back to the woman’s uterus. Fourteen days later, the pregnancy may be confirmed using a blood or urine pregnancy test.

The average success rate for IVF is around 30% from one attempt, 60-70% from three attempts, and approaches 90% with 6 attempts. For comparison, and in the absence of fertility problems, the pregnancy rate for a healthy couple trying to conceive naturally is around 10% per month.
